This Privacy Policy describes how Silence Medical, PLLC ("Silence Medical," "we," "us") collects, uses, and shares information through our website at silencemed.com (the "Site"). This policy applies to information collected through the Site, including information from prospective patients and visitors who have not yet established a provider-patient relationship.
Important. Once you become a patient, your protected health information ("PHI") is also governed by our Notice of Privacy Practices under the Health Insurance Portability and Accountability Act ("HIPAA"). The Notice of Privacy Practices controls how we use and disclose PHI and describes your rights with respect to that information. The Notice is available at silencemed.com and is provided to you at the start of patient care. Where this Privacy Policy and the Notice of Privacy Practices conflict, the Notice of Privacy Practices controls as to PHI.
1. Information We Collect
1.1 Information You Provide
When you visit the Site, contact us, or schedule a visit, we may collect information you choose to provide, including your name, email address, phone number, mailing address, date of birth, payment information, and information about your medical history or reason for contacting us.
1.2 Information Collected Automatically
When you use the Site, we and our service providers may automatically collect technical information, including IP address, device type, browser type, pages viewed, the date and time of your visit, and referring URL. We use commonly accepted technologies, including cookies and similar tools, to collect this information.
1.3 Cookies
Cookies are small files placed on your device. We use cookies to enable basic Site functionality, remember your preferences, and measure how visitors use the Site. You can set your browser to refuse cookies; if you do, parts of the Site may not function correctly.

These providers are contractually obligated to protect your information and to use it only for the services they perform for us.
Legal compliance. When required by law, regulation, court order, subpoena, or to protect rights, property, or safety.
Business transfers. In connection with a merger, acquisition, financing, or sale of assets, with appropriate confidentiality protections.
With your consent. When you direct us to share information.
4. Payment Information
We do not store full credit-card or bank-account numbers. Payment processing is handled by a PCI-compliant third-party processor. We receive only limited information about your payment, such as the last four digits of the card, the brand, and authorization status.
5. Security
We use administrative, technical, and physical safeguards designed to protect the information we collect. Electronic transmissions of health information are encrypted in transit. No method of electronic transmission or storage is completely secure, and we cannot guarantee absolute security.
6. Your Choices
You may decline to provide information, although doing so may limit your ability to use the Services. You may opt out of marketing communications by following the instructions in the message or by contacting us. You may set your browser to refuse cookies. 7. Children’s Privacy
The Site and Services are not directed to children under 13. We do not knowingly collect personal information from children under 13. If you believe a child has provided information to us, please contact us so we can delete it.
8. Out-of-State Visitors
Our Services are limited to patients in Arizona and Colorado. If you visit the Site from another state or country, you understand that information you provide may be processed in the United States and that U.S. law will apply.
9. Changes to This Policy
We may update this Privacy Policy from time to time. The version in effect when you visit the Site governs your visit. Material changes will be posted at silencemed.com with an updated effective date.
